Job Title- RN Care Manager - ED

Location - New York, NY 10004

Duration - 8 weeks (This is initially a contract; however, the client might extend the contract or hire permanently based on performance and attendance.)

Shift- Day shift

Pay - $60/hr on W2

Clinical Background

• RN (Registered Nurse) license - very likely required for this type of MLTC/MAP Care Manager role
• Experience in:
• Care management
• Case management
• Managed care
• Home care
• Community health
• Utilization review
• Discharge planning

Knowledge & Technical Skills

• Understanding of:
• MLTC (Managed Long-Term Care)
• MAP (Medicaid Advantage Plus)
• UAS assessments
• Care planning
• Medicare/Medicaid regulations
• Ability to:
• Develop individualized care plans
• Perform risk stratification
• Coordinate multidisciplinary care
• Review clinical assessments
• Identify gaps in care

Communication & Coordination

• Strong communication with:
• PCPs (Primary Care Physicians)
• Members/patients
• Families/caregivers
• Nurses and interdisciplinary teams
• Ability to:
• Handle member complaints
• Coordinate services across providers
• Educate members
• Conduct monthly follow-ups
